As an LCM, you will be the central point of contact for clients, orchestrating their care journey and ensuring seamless coordination across a multitude of providers and services.
- • In this pivotal remote role, you will report to the Director of Enhanced Care Management, ECM Clinical Manager, and/or ECM Program Manager. Your primary responsibility will be to serve as the client’s trusted advocate and primary liaison, working collaboratively with their entire care team. This includes physicians, specialists, pharmacists, social service providers, and other essential support systems. Your expertise will ensure that all parties are aligned on the client's needs and that their care plan is comprehensive, effective, and person-centered.
- • A key aspect of your role involves meticulous case management. You will oversee the development and implementation of individualized care plans, setting achievable goals in collaboration with the client and their support network. This includes assessing a wide spectrum of member needs, encompassing physical health, mental health, substance use disorders (SUD), oral health, palliative care, memory care, and trauma-informed care. You will also identify and address critical social determinants of health, such as housing instability and lack of access to essential community-based services and supports.
- • You will be empowered to offer services wherever the member resides, seeks care, or finds most accessible. This flexibility allows for a hybrid approach, including office-based, telehealth, or field-based services, ensuring that care is delivered in the most convenient and effective manner for the client. A significant part of your work will involve connecting clients to vital social services and community resources, acting as a bridge to support systems that promote independence and well-being.
- • Advocacy is at the heart of this role. You will champion the client's needs with healthcare professionals, including their Primary Care Physician (PCP) and other specialists, ensuring their voice is heard and their preferences are respected. You will leverage evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles, to foster engagement, promote positive health behaviors, and build resilient relationships.
- • Proactive outreach and engagement are crucial for identifying and enrolling eligible individuals into the Enhanced Care Management (ECM) program. You will meticulously log all outreach activities within the Client Relationship Management (CRM) system, maintaining accurate records of your interactions and progress.
- • Continuous evaluation of client progress is essential. You will regularly assess the effectiveness of the care plan, update SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals, and make necessary adjustments to ensure optimal outcomes. Providing mental health promotion and support will be an integral part of your approach, contributing to the holistic well-being of each client.
- • You will be responsible for coordinating practical support, such as arranging transportation services (e.g., ACCESS), to ensure clients can attend appointments and access necessary resources. Meticulous and timely documentation is paramount. You will complete all required documentation, including outcome measures, within established timeframes, ensuring compliance with program guidelines and maintaining up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems.
- • Furthermore, you will contribute to program oversight by completing monthly reports that ensure program compliance and identify areas for improvement. Continuous learning is encouraged, and you will be expected to attend assigned training sessions to stay abreast of best practices and evolving healthcare landscapes. 🎯 Requirements
- • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
- • Experience working in a remote setting.
- • Working knowledge of government and community resources related to social determinants of health.
- • Excellent oral and written communication skills and positive interpersonal skills.
- • General computer skills and a working knowledge of Google Workspace, MS Office, and the internet.
- • Willing and able to work Monday-Friday 8:30 am - 5:00 pm Pacific Time, with the possibility of evenings and weekends.
