  • • The Clinical Manager at Fira Health, Inc. is a pivotal leadership role responsible for ensuring the quality, compliance, and continuity of patient care in a home health setting. This position directly impacts patient outcomes by overseeing care planning, documentation accuracy, and interdisciplinary coordination, all while adhering to Medicare Conditions of Participation and OASIS standards.
  • • The role is essential to maintaining operational excellence and regulatory compliance, serving as a bridge between clinical staff, physicians, schedulers, and leadership to ensure seamless, high-quality home health services.
  • • Day-to-day responsibilities include coordinating and overseeing care planning for all patients under the caseload, reviewing new referrals for clinical appropriateness, and approving and co-signing clinical orders, including those for LVNs and follow-up visits.
  • • The Clinical Manager provides guidance on OASIS documentation, assists RNs with utilization reviews, and facilitates case conferences and interdisciplinary communication to ensure comprehensive care planning and timely interventions.
  • • They ensure timely submission of assessments and completion of documentation in alignment with billing and compliance guidelines, while maintaining consistent communication with physicians, schedulers, therapists, and field staff.
  • • Supporting the Director of Nursing, the Clinical Manager supervises field staff as needed, assists with scheduling, participates in call rotation, and contributes to triage responsibilities to uphold 24/7 coverage and responsiveness.
  • • They monitor patient status, skilled need, and homebound status to confirm ongoing eligibility and coordinate corrections with the QAPI team to ensure accurate, timely documentation and continuous quality improvement.
  • • The Clinical Manager works primarily in an office-based environment with occasional field responsibilities, requiring local travel and patient home visits as needed, and must be able to manage multiple priorities in a fast-paced setting.
  • • This role offers the opportunity to develop advanced leadership and clinical operations expertise in home health, deepen proficiency in Homecare Homebase EHR systems, and contribute to quality improvement initiatives that elevate patient care standards.
  • • Success in this position can lead to greater influence in clinical program development, enhanced regulatory compliance knowledge, and preparation for senior leadership roles within home health or post-acute care organizations.

🎯 Requirements

  • • Current and active Registered Nurse (RN) license in the state of Texas
  • • Minimum of one (1) year of recent clinical experience as a Registered Nurse
  • • At least one (1) year of experience in a home health setting
  • • Proficiency with Homecare Homebase EHR system for managing patient documentation and workflows
  • • Valid driver’s license and reliable transportation for local travel and field visits
