📋 Description

• • Provide direct nursing care to patients in their homes, assessing and recording vital signs, monitoring patient conditions, and documenting health status accurately.
• • Administer treatments including dispensing medications, changing bandages, and managing oxygen supply and equipment as prescribed by healthcare providers.
• • Educate patients and family members on assisted living techniques such as using canes, walkers, special utensils for eating, personal hygiene protocols, and safe lifting and repositioning methods.
• • Support patients with disabilities, mental illnesses, or challenging behaviors by applying evidence-based care strategies tailored to individual needs.
• • Operate independently in variable and sometimes ambiguous patient care situations, making clinical judgments aligned with established guidelines and organizational objectives.
• • Coordinate care within the OneHome integrated post-acute care model, collaborating with physicians, hospitals, and health plans to ensure continuity of services for Humana members.
• • Perform home visits on weekends to meet patient care demands, ensuring consistent access to nursing services outside standard business hours.
• • Maintain strict compliance with OSHA, HIPAA, HIV/AIDS, Alzheimer’s, Medical Errors, and Domestic Violence certification requirements during all patient interactions.
• • Adhere to all state and federal regulatory standards for home health nursing, including accurate documentation and reporting procedures.
• • Participate in Humana’s Tuberculosis (TB) screening program as a condition of employment, with ongoing compliance required.
• • Maintain a valid Florida RN license and current CPR certification to remain eligible for patient-facing clinical duties.
• • Safely operate a personal vehicle to travel between patient residences, ensuring compliance with Humana’s Driver Safety Program requirements.
• • Demonstrate proficiency in navigating diverse home environments while maintaining professionalism, empathy, and adherence to patient confidentiality.
• • Follow Humana’s cultural values of inclusivity, care, and fairness in all interactions with patients, families, and care teams.
• • Contribute to improving health outcomes for Medicare and Medicaid beneficiaries through personalized, home-based care delivery.
• • Work within a value-based care framework that prioritizes patient well-being, reduced hospital readmissions, and coordinated post-acute services.
• • Utilize clinical judgment to adapt care plans based on evolving patient conditions, with minimal supervision but under established protocols.
• • Ensure all equipment used in patient care is properly maintained, sanitized, and used according to clinical and safety standards.
• • Report changes in patient status promptly to supervising clinicians and care coordinators to facilitate timely interventions.
• • Engage in continuous learning and professional development to maintain certifications and enhance clinical competency.
