- • Provide skilled occupational therapy services to patients in their homes across Natomas, California, and surrounding counties including Petaluma, San Francisco, San Mateo, Santa Clara, Santa Cruz, Alameda, Contra Costa, Solano, Napa, Yolo, Placer, El Dorado, and Sacramento.
- • Conduct comprehensive in-home evaluations to assess patients’ functional abilities, safety risks, and therapeutic needs related to daily living activities.
- • Develop individualized treatment plans in collaboration with Case Managers to improve patients’ independence in activities such as dressing, bathing, cooking, and mobility.
- • Implement interventions focused on home safety modifications, wheelchair seating and positioning, pain management, falls prevention, and management of chronic diseases and dementia.
- • Educate patients and family caregivers on therapeutic techniques, adaptive equipment use, medication management, and strategies to maintain safety and function in the home environment.
- • Serve as a patient advocate by coordinating with interdisciplinary care teams including nurses, physical therapists, and social workers to ensure holistic, patient-centered care.
- • Document all patient assessments, progress, and interventions in compliance with clinical, regulatory, and insurance requirements.
- • Travel to patient homes daily to deliver in-person care; remote or work-from-home positions are not available unless explicitly stated.
- • Participate in ongoing professional development opportunities provided by 21HHS to maintain clinical excellence and stay current with best practices in home health occupational therapy.
- • Contribute to 21HHS’s mission of delivering compassionate, family-centered care by modeling empathy, professionalism, and dedication in every patient interaction.
- • Maintain consistent communication with Case Managers to align on patient goals, adjust care plans, and report changes in patient status or home environment.
- • Adhere to all agency policies, clinical protocols, and state/federal regulations governing home health services.
- • Help sustain 21HHS’s industry-leading hospital readmission rates under 10% by delivering high-quality, evidence-based interventions that reduce risk and promote long-term stability.

🎯 Requirements
- • Valid California Occupational Therapist license
- • Current CPR certification
- • Valid driver’s license and reliable transportation for in-home patient visits
- • Experience providing home health occupational therapy services
- • Ability to work independently in diverse home environments
- • Strong documentation and communication skills
