📋 Description
- • Provide comprehensive physical therapy evaluations and treatment plans for patients in their homes across sub-assigned geographic territories in Southwest Tucson.
- • Assess patient functional abilities and limitations to determine the need for intervention and establish measurable, patient-centered treatment goals in collaboration with patients and caregivers.
- • Develop and implement individualized therapeutic plans that address physical, social, spiritual, cultural, educational, and age-specific patient needs.
- • Deliver skilled, evidence-based physical therapy interventions for a wide variety of clinical conditions including post-op orthopedic recovery, neurorehabilitation, and cardiac rehabilitation.
- • Document all patient interactions, progress, and treatment modifications in accordance with regulatory standards and departmental timeframes using the Home Care Home Base electronic medical record system.
- • Utilize a company-provided tablet and cell phone for documentation and communication while maintaining compliance with professional practice guidelines.
- • Reassess treatment goals regularly and modify care plans based on patient progress or lack thereof, ensuring continuity of care and optimal outcomes.
- • Communicate effectively with physicians, nurses, interdisciplinary teams, patients, and family members to report progress, clarify orders, and coordinate care.
- • Defend clinical decisions with standard practices and current literature, and proactively contact physicians to resolve complex patient care issues.
- • Participate in discharge planning and recommend modifications to referring physician orders as clinically indicated.
- • Supervise and delegate tasks to Physical Therapy Assistants (PTAs), Athletic Trainers (ATCs), therapy technicians, and students in compliance with departmental policies and regulatory requirements.
- • Assist in the orientation and training of new staff members and ensure competency of delegated personnel prior to task assignment.
- • Attend required departmental meetings and actively participate in process improvement initiatives to enhance patient care quality and operational efficiency.
- • Maintain adherence to productivity standards set by the department while managing caseloads efficiently within designated travel territories.
- • Demonstrate adaptability in scheduling and assignments, including assisting in other patient care areas as needed to support team needs.
- • Perform all duties in alignment with state and national regulatory standards, as well as Banner Health’s established departmental policies and procedures.
- • Maintain current BLS certification and a valid Physical Therapy license in the State of Arizona.
- • Obtain and maintain an Arizona Fingerprint Clearance Card throughout employment.
- • Reimbursement for miles driven during patient visits is provided as part of the role.
- • Work a full-time schedule Monday through Friday, 8:00 AM to 5:00 PM, with day shift hours.
🎯 Requirements
- • Graduation from an APTA-accredited Physical Therapy program or equivalent
- • Current Physical Therapy license in the State of Arizona
- • Current BLS certification
- • Arizona Fingerprint Clearance Card
- • Proficiency with electronic medical record systems and standard patient care equipment
- • Ability to drive and travel within designated geographic territories
