- • As a Lead Care Manager at Vynca, you will serve as the primary point of contact for individuals with complex health and social needs, ensuring seamless coordination between medical providers, social services, and community resources to improve quality of life and promote independence at home. - • You will conduct holistic assessments across physical health, mental health, substance use, oral health, palliative and memory care, trauma-informed needs, housing, and social supports, then develop and oversee personalized care plans with SMART goals that are regularly evaluated and updated based on member progress and evolving needs.
- • Your day-to-day responsibilities include providing care in the member’s preferred setting—whether via telehealth, office visits, or field-based outreach—connecting clients to essential social services, arranging transportation (e.g., ACCESS), advocating with healthcare providers, and utilizing evidence-based practices like Motivational Interviewing and Harm Reduction to foster engagement and self-efficacy.
- • You will maintain accurate, timely documentation in both Electronic Medical Record (EMR) and Customer Relationship Management (CRM) systems, complete monthly compliance reports, participate in required trainings, and collaborate closely with caregivers, family support persons, and interdisciplinary care teams to ensure alignment and continuity of care.

🎯 Requirements
- • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations, including experience with complex medical and social needs.
- • Willingness and ability to work Monday–Friday, 8:30am–5:00pm Pacific Time, with flexibility for occasional evenings and weekends as needed to meet member needs.
- • Working knowledge of government and community resources related to social determinants of health, including housing, food security, transportation, and benefits programs.
- • Clean driving record, valid driver’s license, and reliable transportation for potential field-based visits.
- • Excellent oral and written communication skills, with the ability to convey complex information clearly and compassionately to diverse audiences.
- • Positive interpersonal skills essential for building trust with clients, families, and interdisciplinary care teams.
- • General computer proficiency and working knowledge of Google Workspace, MS Office, and internet-based systems for documentation and reporting.
- • Bilingual proficiency in English and Spanish is preferred but not required.
