  • • Conduct concurrent and retrospective reviews of patient medical records to ensure accurate documentation of diagnoses, procedures, and severity of illness for all payors.
  • • Collaborate with physicians, mid-level providers, ancillary staff, and medical coders to identify and clarify principal and secondary diagnoses and procedures.
  • • Initiate and manage provider queries to obtain clarification on clinical documentation, ensuring compliance with Medicare rules, regulations, and guidelines.
  • • Utilize EHR tracking systems to document findings, track documentation gaps, and monitor resolution of clinical documentation issues.
  • • Maintain current knowledge of federal and state coding compliance standards, including CMS guidelines and ICD-10-CM/PCS coding requirements.
  • • Provide ongoing education and feedback to clinical staff on documentation best practices, including specialty-specific guidance in collaboration with the Physician Advisor.
  • • Attend provider service line meetings and team huddles to serve as a documentation resource and promote awareness of documentation opportunities.
  • • Develop and distribute educational materials such as tip sheets, presentations, and training resources to support clinical documentation improvement across the organization.
  • • Support Lean principles by identifying inefficiencies in documentation processes and championing continuous improvement initiatives.
  • • Ensure all documentation activities comply with organizational policies, federal regulations, and corporate integrity standards.
  • • Maintain strict confidentiality and professionalism when handling sensitive patient health information.
  • • Deliver high-quality customer service to clinical staff while managing high-volume documentation reviews with attention to detail and accuracy.
  • • Work independently and manage multiple priorities in a productivity-driven environment with minimal supervision.
  • • Adapt to frequent changes in coding guidelines, regulatory requirements, and health system processes.
  • • Virtually attend meetings, trainings, and business functions during Pacific Time Zone business hours.
  • • Perform all duties with a commitment to St. Charles Health System’s values of accountability, caring, and teamwork.
  • • Wear required Personal Protective Equipment (PPE) as needed for occasional in-person interactions or site visits.
  • • Maintain focus during extended periods of screen-based medical record review, typically 75% or more of the workday.
  • • Operate efficiently using EPIC and 3M 360 Encompass software for documentation tracking and analysis.
  • • Support organizational vision and mission by contributing to improved patient care through accurate clinical documentation.

🎯 Requirements

  • • Current Oregon RN License
  • • CCDS (Certified Clinical Documentation Specialist) or CDIP (Certified Documentation Improvement Practitioner) certification
  • • Minimum of two (2) years recent Clinical Documentation Integrity Specialist (CDIS) experience
  • • Proficient use of EPIC and 3M 360 Encompass software (preferred)
  • • Ability to work remotely from an approved state: Oregon, Arizona, Arkansas, Florida, Idaho, Missouri, Montana, Nevada, New Mexico, North Carolina, Oklahoma, Tennessee, Utah, or Wisconsin
  • • Ability to work Pacific Time Zone business hours (8am - 4:30pm, Monday-Friday)
