- • The Community Outreach Specialist at Vynca plays a vital role in expanding access to care for individuals with complex needs by building and strengthening relationships with community partners across Sacramento County. This position is essential to advancing Vynca’s mission of providing comprehensive care for more quality days at home, particularly through its Enhanced Care Management programs.
- • Day to day, the specialist will build and maintain relationships with community stakeholders such as resource centers, medical facilities, and advocacy groups; drive referrals into Vynca care programs; develop and execute strategic marketing plans; track sales activities and conduct presentations; support community events; serve as a liaison between patients and practices; and strengthen referral partnerships through consistent outreach efforts.
