- • A key aspect of your role will involve supporting the care team by identifying member-specific community support needs and providing appropriate health education to empower members in their health journey.
- • You will actively coordinate and promote awareness of available community outreach resources, ensuring members are informed about the services and support systems accessible to them.
- • Serving as a primary point of contact, you will address member inquiries regarding community and care resources, guiding them towards beneficial opportunities.
- • This role requires meticulous documentation and maintenance of community resource information, ensuring all records adhere to established standards of practice and health plan policies.
- • You will provide essential assistance to the clinical team, including nurses and social workers, through activities such as outreach, community education, offering informal guidance, and providing direct member support.
- • A significant responsibility includes conducting non-clinical general health assessments to accurately refer members to appropriate care and services, effectively resolving concerns on their behalf, and gathering critical information for medical providers and internal staff.
- • Furthermore, you will perform non-medical assessments focusing on areas such as home safety, evaluation of the community environment and available resources, transportation needs, employment status, and other relevant factors to facilitate referrals and gather comprehensive member information.
- • You will be tasked with conducting telephonic and/or in-person outreach to engage and locate individuals and families within the community who may be difficult to reach, ensuring no one falls through the cracks.
- • This may involve making visits to individual homes and engaging with community organizations to build relationships and identify new resource opportunities.
- • A strong understanding of Social Determinants of Health (SDOH) barriers is essential for effectively assessing member needs and connecting them with appropriate support.
- • You will perform other duties as assigned, demonstrating flexibility and a willingness to contribute to the team's success.
- • Adherence to all company policies and standards is paramount to ensure operational integrity and member satisfaction.
- • The ideal candidate will possess excellent customer service and communication skills, enabling them to build rapport and effectively assist members from diverse backgrounds.
- • Proficiency in Microsoft Applications is required for daily tasks, including documentation, communication, and data management.
- • A significant portion of this role (approximately 50%) involves fieldwork, requiring travel within the designated community to meet with members and organizations.
- • Bilingual proficiency in Spanish is highly desirable, allowing for broader reach and more effective communication with a diverse member population.
- • The ability to conduct thorough assessments, both clinical and non-clinical, is fundamental to identifying member needs and facilitating appropriate referrals.
- • Experience with telephonic outreach and engagement is necessary for connecting with members remotely.
