- • This role is a Diabetes Specialty Care Nurse Practitioner (APRN) focused on delivering world-class virtual diabetes care to vulnerable seniors through Devoted Health’s next-generation virtual specialty clinic, expanding access to specialized chronic condition management.
- • Day-to-day responsibilities include conducting virtual patient assessments, ordering and interpreting diagnostics, developing individualized treatment plans, managing medications, collaborating with an interprofessional team (including physicians, CDCES, pharmacists, and social workers), providing clinical escalation support, participating in panel reviews, maintaining accurate EHR records, and contributing to quality improvement initiatives.

🎯 Requirements
- • APRN with Master’s or Doctoral degree in Nursing specializing in diabetes care
- • Minimum 5 years of outpatient clinical practice, ideally in endocrinology or diabetes-focused settings
- • At least 2 years of concentrated experience in diabetes management and education
- • Proficiency in telehealth technology and electronic health records (EHR)
- • Multi-state licensure required, with willingness to obtain and maintain additional licenses
- • Active RN, APRN, and BLS certification required at hire and maintained throughout employment
- • Proficiency in English required; Spanish proficiency preferred
