• • We are seeking a highly analytical and operationally-minded Director of Population Health to join our growing team. This pivotal role operates at the nexus of medical economics, data analytics, and healthcare operations. You will be instrumental in identifying critical gaps in care delivery, cost efficiency, and patient outcomes across our network. Your primary responsibility will be to translate these data-driven insights into measurable clinical actions and sustainable operational solutions, working in close collaboration with our Medical Director and dedicated care teams.
• • The ideal candidate possesses a strong foundation in analytical methodologies, a deep understanding of value-based care models, and a proven track record of converting data insights into executable strategies. You will be adept at connecting the intricate relationships between payer contracts, clinical workflows, and financial performance, ultimately driving Chamber Cardio's commitment to delivering superior outcomes at a reduced cost.
• • **Data-Driven Insight & Medical Economics:**
• • Spearhead the development and refinement of comprehensive population health analytics and interactive dashboards. These tools will be crucial for identifying and quantifying opportunities related to cost containment, quality enhancement, and utilization optimization across our patient populations.
• • Conduct in-depth analyses to precisely pinpoint areas of care fragmentation, identify key drivers of high healthcare expenditures, and understand variations in treatment and outcomes among different patient segments and provider groups.
• • Masterfully translate intricate data findings into clear, actionable strategies that effectively inform and guide clinical decision-making and operational priorities.
• • Collaborate closely with the Finance department and the Medical Director to develop robust financial models for evaluating the Return on Investment (ROI) of proposed interventions and to rigorously measure the actual impact of implemented programs.
• • **Operational Execution & Clinical Integration:**
• • Work hand-in-hand with the Medical Director to conceptualize, design, and implement innovative programs specifically tailored to address the identified gaps in care and cost.
• • Translate the strategic goals and requirements of payer contracts and risk-based arrangements into practical, user-friendly workflows that foster patient and provider engagement and drive consistent performance improvements.
• • Act as a critical liaison, coordinating efforts between the analytics team, frontline care teams, and the product development group to ensure that data-driven insights are seamlessly integrated into the daily practices and decision-making processes of our clinical staff.
• • Define, establish, and meticulously track Key Performance Indicators (KPIs) for all program executions, encompassing metrics from patient enrollment and engagement levels to critical clinical outcomes and the overall total cost of care.
• • **Cross-Functional Collaboration & Strategy:**
• • Forge strong partnerships with the Product Management and Data Science teams to enhance and develop the technological tools that effectively surface actionable insights for care teams, making data more accessible and intuitive.
• • Serve as a vital bridge between the analytical perspectives of payers and the practical realities of clinical operations, ensuring a clear alignment with performance targets and contractual obligations.
• • Collaborate strategically with senior leadership to prioritize interventions that promise to deliver significant, dual impact on both clinical quality and financial sustainability.
• • Prepare compelling and data-rich performance narratives for presentation to payers, the board of directors, and internal stakeholders, clearly articulating progress, challenges, and strategic direction.
• • **In Your First 90 Days, You Will Achieve:**
• • Successfully establish and deploy foundational population health analytics dashboards that vividly highlight key opportunities in cost, quality, and patient engagement.
• • Identify and clearly define the top 3–5 most critical care or cost gaps present within Chamber Cardio’s active patient populations, and collaboratively develop initial action plans with the Medical Director to address them.
• • Define and implement a clear, standardized framework for accurately measuring the ROI and overall impact of our clinical programs.
• • Deliver a comprehensive, executive-level report detailing early population health insights, the interventions currently underway, and initial performance indicators.
