Most importantly, patients who require follow-up care might not receive it in a timely manner. This role is designed to fundamentally change that trajectory.
• • The Head of Continuity will operate with the autonomy and vision of a startup founder, leading a dedicated, cross-functional team with a clear mission and a rigorous operating model. This leader will be responsible for integrating product development, engineering efforts, clinical insights, implementation processes, and go-to-market strategies into a cohesive and high-performing business unit.
