- • Lead and deliver exceptional patient care as a Medical Director and Attending Physician within Theoria Medical's innovative post-acute care network, specifically serving skilled nursing facilities (SNFs) in the Indianapolis, IN area.
- • Embrace a patient-centered approach, fostering long-term relationships with residents and building trust through consistent presence and high-quality medical services.
- • Take ownership of patient care within assigned SNFs, acting as the primary attending physician responsible for diagnosing, treating, and managing a wide spectrum of acute and chronic conditions.
- • Collaborate closely with a dedicated team of Nurse Practitioners (NPs) and Physician Assistants (PAs), providing essential clinical supervision and guidance to ensure comprehensive and coordinated patient management.
- • Drive the implementation and success of Theoria’s key quality initiatives, including Chronic Care Management, Transitional Care programs, and Remote Patient Monitoring, contributing to improved patient outcomes and operational efficiency.
- • Leverage advanced technology designed to enhance clinical workflows and reduce administrative burden, including an intuitive Electronic Health Record (EHR) system, voice dictation capabilities, and an AI Scribe for efficient and accurate documentation.
- • Access UpToDate® directly within the EHR for immediate clinical decision support, ensuring evidence-based care at the point of service.

🎯 Requirements
- • MD or DO degree from an accredited medical school.
- • Board-certified or board-eligible in Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine.
- • Active and unrestricted U.S. medical license.
- • Experience in post-acute care or geriatrics is a plus.
- • Strong clinical judgment and diagnostic skills.
- • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare teams.
