Your role is pivotal in transforming overwhelmed and potentially skeptical families into confident, engaged clients, ensuring they receive the comprehensive geriatric care management services Hera provides.
  • • You are the embodiment of Hera's commitment to compassionate and competent care. When families are referred, they often feel lost, confused by healthcare systems, and unaware of how services like geriatric care management, especially those covered by Medicare, can benefit them. Your primary objective is to cut through this confusion, build immediate trust, and make the path to receiving care clear and accessible.
  • • This position is a unique blend of relationship-building, operational excellence, and a touch of salesmanship. You are not just processing applications; you are actively engaging with individuals during a vulnerable time, understanding their unique needs, and demonstrating how Hera can be the solution. Your success is measured by your ability to remove all obstacles, ensuring a seamless transition from referral to a scheduled appointment with their dedicated "Hero."
  • • You will own the entire onboarding journey for each family. This means meticulously managing paperwork, verifying documentation, confirming insurance coverage, and scheduling initial consultations. You will proactively address any potential roadblocks, whether they stem from missing information, skepticism from adult children, or technological barriers faced by the patient.
  • • A key aspect of your role involves leveraging technology to create an efficient and positive onboarding experience. Instead of relying on outdated methods like faxes and voicemails, you will utilize modern tools to streamline communication, document submission, and appointment scheduling, making the process as frictionless as possible.
  • • You will act as a vital liaison between families and Hera's internal care team. This involves coordinating effectively to ensure a smooth and informed handoff to the assigned "Hero," providing them with all necessary context to continue the family's care journey seamlessly.
  • • Furthermore, you will assist families in navigating various technological platforms. This includes guiding them through document uploads, facilitating their participation in telehealth appointments, and ensuring they can confidently sign electronic forms, thereby empowering them to engage with Hera's services.
  • • A critical responsibility is to identify and address any points of friction within the intake and onboarding process. 🎯 Requirements

  • • Proven experience in healthcare sales, care coordination, customer success, or a similar client-facing role, preferably with experience supporting older adults or their families.
  • • Exceptional communication and interpersonal skills, with the ability to build rapport and trust quickly over the phone and through other communication channels.
  • • Strong organizational skills and a meticulous attention to detail, ensuring that no task or piece of information falls through the cracks.
  • • A proactive and problem-solving mindset, with a knack for identifying inefficiencies and developing streamlined solutions.
  • • Tech-savviness and comfort with using various digital tools and platforms for communication, documentation, and scheduling.
