- • Provide direct, evidence-based nursing care to patients in their homes, ensuring comfort, safety, and continuity of care throughout their health journey.
- • Conduct comprehensive patient assessments, develop individualized care plans, and implement interventions tailored to each patient’s unique medical and emotional needs.
- • Administer medications, perform injections, manage IV therapies, and monitor patient responses with precision and clinical judgment.
- • Perform specialized wound care, including dressing changes, infection control, and healing progress tracking, in home environments.
- • Educate patients and their caregivers on disease management, medication regimens, symptom monitoring, and emergency response protocols.
- • Utilize digital tools and electronic health records to accurately document patient care, update treatment plans, and communicate with interdisciplinary teams.
- • Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate holistic, patient-centered care.
- • Maintain autonomy in clinical decision-making while receiving ongoing support and guidance from a dedicated home health team.
- • Travel to patient residences across the St. George region, requiring reliable transportation and adherence to a safe driving record.
- • Adapt to variable shift schedules, including every other weekend availability, to ensure consistent patient coverage and care continuity.
- • Lift, transfer, and ambulate patients, as well as handle medical equipment and supplies weighing up to and including heavy items.
- • Navigate diverse home environments, often with physical obstacles such as clutter, cords, or limited space, while maintaining patient safety and professional standards.
- • Communicate clearly and compassionately with patients, families, and colleagues, both verbally and in writing, to ensure accurate information exchange and emotional support.
- • Demonstrate manual dexterity to operate medical devices, prepare medications, and use computers for documentation and data entry.
- • Remain vigilant during emergency situations, quickly recognizing and responding to changes in patient condition or environmental hazards.
- • Maintain current licensure, certifications, and continuing education requirements to uphold clinical excellence and regulatory compliance.
- • Commit to professional growth by pursuing a Bachelor of Science in Nursing (BSN) if hired with less than 12 months of RN experience, completing it within four years of start date.
🎯 Requirements
- • Current RN License in the state of Utah
- • Basic Life Support (BLS) Certification for healthcare providers
- • Valid driver’s license, insured and reliable transportation, and an acceptable driving record
- • Bachelor’s Degree in Nursing (BSN) required as of April 27, 2025; those hired with less than 12 months of RN experience must obtain BSN within four years
- • Ability to communicate effectively both verbally and in writing
- • Physical ability to lift, transfer, and ambulate patients and handle equipment up to heavy weights
