- • Deliver direct patient care in home settings as a Registered Nurse, conducting routine home health visits to assess, plan, and implement individualized care plans.
- • Serve as a specialized wound care resource by performing comprehensive wound assessments, developing evidence-based care plans, and providing consultation to interdisciplinary teams and patients’ families.
- • Utilize clinical expertise to optimize healing outcomes for patients with acute and chronic wounds, ostomies, and related conditions through evidence-based interventions and patient education.
- • Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate holistic, patient-centered care aligned with clinical goals and patient preferences.
- • Maintain accurate, timely documentation of patient assessments, interventions, and outcomes using electronic health records and other digital systems.
- • Educate patients and caregivers on wound care techniques, infection prevention, medication management, and self-care strategies to promote independence and reduce hospital readmissions.
- • Advocate for patient safety and quality outcomes by identifying risks, implementing preventive measures, and adhering to clinical protocols and regulatory standards.
- • Participate in ongoing professional development and team training to stay current with best practices in home health and wound care.
- • Mentor and guide other nursing staff through clinical support, knowledge sharing, and performance feedback to enhance team competency and patient care quality.
- • Manage logistics of home visits including scheduling, route planning, and coordination with case managers to ensure efficient and timely service delivery.
- • Transport and utilize medical equipment and supplies during home visits, ensuring proper handling, storage, and disposal according to infection control guidelines.
- • Respond to urgent patient needs during home visits, including recognizing and managing clinical emergencies with appropriate interventions and communication.
- • Maintain compliance with state nursing practice laws, Medicare/Medicaid regulations, and organizational policies governing home health services.
- • Demonstrate cultural competence and compassion when engaging with diverse patient populations across rural and urban communities in Southern Utah.
- • Balance autonomy in clinical decision-making with accountability to supervisory leadership and quality assurance protocols.
- • Contribute to continuous improvement initiatives by providing feedback on workflows, equipment needs, and patient care processes to enhance operational efficiency and clinical outcomes.
🎯 Requirements
- • Current license to practice as a Registered Nurse (RN) in the state of practice
- • Bachelor’s degree in nursing (BSN) from an accredited institution (must be obtained within four years of hire or promotion)
- • Current Basic Life Support (BLS) Certification for Healthcare Providers
- • One year of clinical nursing experience
- • Demonstrated experience in assigned clinical area
- • Valid driver’s license, insured and reliable transportation, and an acceptable driving record
