- • Serve as a Hospice Case Manager RN responsible for assessing, planning, implementing, and evaluating individualized care plans for patients in their homes.
- • Utilize the nursing process with evidence-based practices, advanced technology, and therapeutic interventions to deliver high-quality end-of-life care.
- • Coordinate seamless care across an interdisciplinary healthcare team, ensuring continuity from patient admission through discharge.
- • Act as the primary point of contact for patients and their families, providing emotional support, education, and guidance during the hospice journey.
- • Advocate for patients by navigating complex healthcare systems and connecting them with essential resources and services.
- • Manage patient care logistics including medication administration, symptom management, and equipment coordination within the home setting.
- • Maintain accurate and timely documentation of patient care through computer systems, including assessments, interventions, and outcomes.
- • Collaborate with physicians, social workers, chaplains, aides, and payors to ensure comprehensive, patient-centered care.
- • Conduct home visits to evaluate patient conditions, adjust care plans, and provide hands-on clinical support as needed.
- • Ensure compliance with regulatory standards and organizational policies for hospice and home health services.
- • Lead patient and family education on disease progression, symptom management, and end-of-life care options.
- • Manage caseloads efficiently to optimize resource use while maintaining high standards of care quality and patient satisfaction.
- • Participate in team meetings and care conferences to discuss patient progress and coordinate interdisciplinary interventions.
- • Maintain a current driver’s license, reliable transportation, and an acceptable driving record to perform home visits across the service area.
- • Perform physical tasks including lifting, transferring, and ambulating patients; bending to retrieve supplies; and operating medical equipment with precision.
- • Communicate effectively both verbally and in writing with patients, families, and healthcare professionals under varying conditions, including emergencies.
- • Work flexible 8AM–5PM shifts with rotating holidays to ensure consistent patient coverage and work-life balance.
- • Demonstrate compassion, professionalism, and clinical excellence while supporting patients and families through sensitive end-of-life transitions.
🎯 Requirements
- • Current RN License in the state of practice
- • Basic Life Support (BLS) Certification for healthcare providers
- • Current driver’s license, insured and reliable transportation, and an acceptable driving record
- • Ability to communicate effectively both verbally and in writing
- • RNs with less than 12 months of experience must obtain a BSN within 4 years of hire
