- • The Case Management Nurse role at Chamber Cardio, Inc. is critical to advancing the company’s mission of transforming heart health by ensuring patients receive timely, coordinated, and evidence-based care across the continuum. As an RN Case Manager, you will directly impact patient outcomes by reducing delays, preventing avoidable readmissions, and supporting seamless transitions from hospital to home.
- • Day to day, you will review patient cases within 24–48 hours of admission or clinical triggers, conduct comprehensive assessments to identify clinical, social, and system barriers, and collaborate with providers and interdisciplinary teams to develop and execute individualized care plans. You will facilitate safe discharges, coordinate post-discharge follow-up and referrals, monitor key metrics like length of stay and avoidable delays, and maintain accurate documentation while communicating clearly with patients, families, and care teams.
