- • As a Lead Care Manager at Vynca, you will serve as the primary point of contact for clients with complex needs, ensuring coordinated, client-centered care across physical health, mental health, social determinants, and community resources.
- • You will assess member needs, develop and update individualized care plans using SMART goals, provide advocacy and education, and connect clients to essential services such as housing, transportation, and community-based supports.
- • You will work in a hybrid model — conducting in-field visits, telehealth sessions, and office-based work — while maintaining accurate records in EMR and CRM systems, completing monthly compliance reporting, and participating in required training.
- • You will collaborate with healthcare providers, specialists, pharmacists, caregivers, and community organizations to promote wellness, recovery, independence, and member empowerment, guided by Vynca’s core values of Excellence, Compassion, Curiosity, and Integrity.

🎯 Requirements
- • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- • Valid driver’s license, clean driving record, and reliable transportation for travel throughout Santa Clara County
- • Excellent oral and written communication skills, with positive interpersonal abilities to engage clients, families, and multidisciplinary care teams
- • Working knowledge of government and community resources related to social determinants of health (e.g., housing, benefits, social services)
- • Proficiency with Google Workspace, MS Office, and general computer skills for documentation, reporting, and CRM/EMR system use
- • Bilingual (English/Spanish) preferred
