📋 Description

• Serve as the primary care provider for adult and pediatric patients within family homes, delivering comprehensive longitudinal primary care, preventive services, acute care, and chronic condition management through in-home visits and telehealth modalities (video, phone, SMS).
• Manage a designated panel of families, coordinating whole-family health through Nest’s NAP (Nest Action Plan), establishing individualized health goals in collaboration with interdisciplinary team members including MDs, LCSWs, RNs, psychiatrists, and dietitians.
• Utilize Nest Health’s electronic health record and care management tools to document patient interactions, track clinical tasks, communicate with team members, and coordinate with community resources.
• Diagnose, treat, and manage chronic conditions such as diabetes, hypertension, CHF, depression, and respiratory illnesses, including prescribing and reconciling appropriate medications.
• Order, administer, obtain, and record diagnostic tests including venipuncture, phlebotomy, and other clinical nursing procedures for both adult and pediatric patients.
• Provide patient education and behavioral health support, ensuring culturally congruent, non-judgmental care that empowers families to understand their conditions and treatment plans.
• Advocate for and navigate families through access to health and social services, including eligibility determination for public programs and community-based resources.
• Participate in daily team huddles and weekly case conferences to review patient progress, discuss complex cases, and align on care coordination strategies across clinical and social support functions.
• Engage in outreach efforts to educate prospective families about Nest’s services, program eligibility, and benefits to expand access to care for high-risk populations.
• Maintain current BLS/CPR certification and comply with all Nest Motor Vehicle Safety Policy requirements, including carrying valid driver’s license, auto insurance, and reliable transportation to reach patient homes.
• Carry portable healthcare supplies during home visits and lift up to 25 pounds as needed while performing in-home clinical duties.
• Work a full-time five-day week with participation in a rotating call schedule that includes occasional evenings and weekend hours each month.
• Collaborate closely with a team of physicians, behavioral health counselors, registered dietitians, and upskilled medical assistants to deliver integrated physical, mental, and social care.
• Adapt to a fast-paced, evolving startup environment with high autonomy, requiring self-directed learning, proactive communication, and the ability to juggle multiple responsibilities in a less structured setting.
• Maintain accurate and timely documentation within the EHR system to support care continuity, compliance, and data-driven improvements in patient outcomes.
• Educate and encourage vaccination against COVID-19 as part of proactive preventive care for Nest families.

🎯 Requirements

• Completion of an accredited Physician Assistant or Nurse Practitioner program with an active, unrestricted license to practice in Arizona
• Minimum 2 years of clinical experience providing care to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions
• Demonstrated proficiency or willingness to train in clinical nursing skills such as venipuncture, phlebotomy, patient assessments, and chronic condition care plan reinforcement
• Current BLS/CPR certification
• Valid driver’s license, personal vehicle, auto insurance, and ability to reliably travel to patient homes across Kingman, Arizona
• Availability to work full-time (five days per week) and participate in a rotating on-call schedule including occasional evenings and weekends
