- • Deliver exceptional, whole-person clinical care to a cohort of complex geriatric patients with chronic conditions, frailty, and high risk of hospitalization, primarily through video telehealth visits.
- • Manage and adjust the clinical trajectory of patients using the 5M framework (Mind, Mobility, Medications, Multicomplexity, and Morality) to align care with patient priorities and clinical needs.
- • Conduct comprehensive assessment visits, including post-acute transition support, diagnosis and disease review, medication reconciliation, and evaluation of quality of care metrics (Stars/Hedis) and social determinants of health.
- • Collaborate closely with patients’ primary care providers, specialists, pharmacists, social workers, community health workers, and family members or caregivers to coordinate integrated, interdisciplinary care plans.
- • Perform ongoing symptom management and chronic disease management tailored to each patient’s clinical and social context, ensuring continuity and responsiveness to emerging barriers.
- • Maintain accurate, timely, and compliant electronic medical records in alignment with legal, ethical, and regulatory standards for telehealth and patient privacy.
- • Adhere strictly to all state and federal regulations governing advanced practice nursing or physician assistant practice, including telehealth licensure and scope-of-practice laws.
- • In rare instances, provide telehealth support to members outside your primary panel to assist other Devoted Medical service lines as needed.
- • Participate in infrequent weekend on-call rotations to support clinical nurses who triage member calls during non-business hours.
- • Ensure all clinical decisions are bias-to-action, driven by patient-centered priorities and multidisciplinary team input.
- • Support the growth and evolution of the Gold Care Program by adapting to new workflows, technologies, and care delivery models in a fast-paced, mission-driven environment.
- • Demonstrate cultural competency and sensitivity when working with vulnerable, high-need older adult populations across multiple geographic regions in the U.S.
🎯 Requirements
- • Active and clear RN and APRN or Physician Assistant license in GA, PA, and/or OH at time of hire, with willingness to obtain and maintain multiple state licenses
- • Three or more years of experience as an Advanced Practice Provider in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care
- • Proficiency in using telehealth technology and electronic health records (EHR)
- • Active BLS certification at time of hire and ability to maintain throughout employment
