- • This is a full-time community-based Advanced Practice Provider role requiring frequent travel within the Pittsburgh, PA territory, focused on conducting comprehensive health risk assessments in members' homes.
- • The incumbent delivers high-quality, patient-centered care through in-home, virtual, and telephonic clinical assessments for members with chronic health conditions, with emphasis on geriatric and palliative populations.
- • The role involves proactive symptom management, advance care planning, and coordination with care teams to support optimal quality of life and health outcomes for members with chronic or life-limiting illnesses.
- • The position is integral to Highmark Health's comprehensive care delivery system, requiring accurate identification and documentation of chronic conditions for risk adjustment and autonomous caseload management in a home-based setting.
- • The person will learn and advance expertise in palliative care, geriatrics, risk adjustment methodologies, population health management, and interdisciplinary care coordination while working autonomously in a high-performing team environment.
🎯 Requirements
- • Graduate from an accredited PA program OR a Nursing Approved Master’s or Post Master’s program
- • 3 years of clinical experience as an NP or PA, preferably in primary care, geriatrics, palliative or hospice care, chronic disease management, home health, or similar community-based setting
- • Current Physician Assistant License OR Current State of PA RN licensure or multi-state licensure through eNLC + CRNP license and Prescriptive Authority in PA
- • CPR Certification and valid driver’s license with reliable transportation and appropriate insurance for travel to patient homes
- • Exceptional communication, motivational interviewing, and empathy skills for patient-centered care and sensitive health discussions

- • Pay range: $102,700.00 minimum to $164,600.00 maximum base pay
