  • • Join Ennoble Care, a leading mobile primary care, palliative care, and hospice service provider, and embody our motto, “To Care is an Honor.” We are seeking a dedicated and experienced Advanced Practice Provider (APP) to deliver exceptional primary care services to homebound and palliative care patients throughout the Salem County, New Jersey region.
  • • As an APP at Ennoble Care, you will play a pivotal role in extending our continuum of care directly to patients in their homes. This mobile model allows for personalized, high-quality healthcare for individuals managing chronic conditions, limited mobility, or requiring specialized palliative support.
  • • Your core responsibility will be to provide comprehensive primary care, focusing on assessing patient health through thorough physical examinations, meticulously obtaining and updating medical histories, and developing individualized, evidence-based care plans. You will be instrumental in diagnosing and treating a diverse patient population, many of whom will have complex chronic diseases.
  • • This role requires a proactive approach to patient management, including developing accurate medical impressions, formulating diagnoses, and prescribing appropriate treatment plans. You will have the autonomy to order diagnostic testing as medically necessary and prescribe medications, supplies, and other interventions to ensure optimal patient outcomes.
  • • A significant aspect of your role will involve fostering strong therapeutic relationships. You will consult with patients, their families, and their caregivers during home visits, building trust and ensuring clear communication about their health status and treatment options.
  • • Effective collaboration and communication are paramount. You will work closely with our office staff and clinical operations teams, utilizing various internal platforms to ensure seamless coordination of care and efficient workflow.
  • • You will be an integral part of our clinical team, attending quarterly medical staff meetings to contribute to the ongoing development and improvement of our care delivery models.

🎯 Requirements

  • • Board Certified with a valid Nurse Practitioner (NP) or Physician Assistant (PA) certification in the state of New Jersey.
  • • Minimum of 1-2 years of experience in primary care, with a strong preference for experience working with geriatric patients.
  • • Demonstrated strong clinical skills, with the ability to accurately and confidently assess, diagnose, and treat patients.
  • • Excellent communication and interpersonal skills, coupled with a positive demeanor and the ability to foster trust with patients, families, and caregivers.
  • • Proven ability to problem-solve effectively and operate independently in a mobile healthcare setting.
