  • • Provide in-person primary care visits as part of a comprehensive longitudinal care model, working in close collaboration with a virtual PCP who serves as the patient’s primary care provider; virtual care modalities may be used as appropriate.
  • • Conduct adult and pediatric wellness visits that address quality care gaps and establish a comprehensive baseline physical exam for all patients.
  • • Provide acute and chronic care requiring physical examination or diagnostic testing, including transition of care visits after high-risk hospitalizations, as needed.
  • • Reconcile and prescribe medications, order and administer labs and diagnostics, and refer to specialist care, as needed.
  • • Partner with an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists and medical assistants upskilled to support social services navigation.
  • • Document care and maintain clear, comprehensive care plans addressing patient and family needs.
  • • Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance.
  • • Participate in community outreach efforts, as needed.
  • • WHAT DO YOU BRING TO THE NEST?
  • • Graduate of an accredited Nurse Practitioner or Physician Assistant program with an active, unrestricted Arizona license.
  • • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions (preferred).
  • • Proven skills, knowledge base, and judgment necessary for independent clinical decision-making and problem-solving.
  • • Hands-on clinical skills, including venipuncture, patient assessment, and chronic care support, or willingness to learn.
  • • Ability to work at least 1-2 days per week (8-10 hours per day), with the opportunity to expand hours as the program grows.
  • • Current BLS/CPR certification.
  • • Passion for serving underserved and vulnerable populations with empathy and respect.
  • • Leadership experience in an interdisciplinary care team preferred.
  • • Startup experience preferred.
  • • WHAT ARE THE TRAVEL REQUIREMENTS?
  • • This is a field-based role, providing in-person care at community facilities or in patient homes across the Kingman service area, transporting health care supplies (up to 25 lbs).
  • • An unrestricted driver’s license, reliable personal vehicle, and auto insurance are required (mileage reimbursed).
  • • Candidates must be willing to follow the Nest’s Motor Vehicle Safety Policy.
