- • The Bilingual (Spanish) Advanced Practice Provider (NP or PA) role in Palliative Care at Devoted Health is critical to delivering compassionate, patient-centered care to members with life-limiting illnesses, ensuring they receive the right care at the right time and place, aligned with the company’s mission to treat members like family.
- • Day to day, the provider will manage a longitudinal panel of 40-50 high-risk patients through telephonic and video encounters, conduct initial assessments, develop palliative plans of care, provide symptom management and goals of care counseling, support care transitions, evaluate hospital admissions for readmission trends, and participate in regular team huddles, interprofessional meetings, and monthly panel reviews.
- • Devoted Health is a rapidly growing, mission-driven healthcare company founded in 2017 that integrates health insurance, clinical care, service, and technology to improve the health and well-being of older Americans, with a virtual Palliative Care program serving members across multiple states who lack access to specialized palliative care.
- • In this role, the provider will deepen expertise in palliative care, lead interprofessional collaboration, contribute to program development in a startup environment, gain experience in virtual multi-state care delivery, and help shape a care model that prioritizes dignity, quality of life, and seamless transitions to hospice, all while working remotely with a supportive, values-driven team.
🎯 Requirements
- • Unrestricted advanced practice license in 1 or more of the Devoted Membership states: FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, PA
- • 5 years in advanced practice, with at least 3 years in Palliative care
- • ACHPN® credential, or ability/willingness to obtain within first six months of employment
- • Willingness to flex between time zones
- • Comfortable with technology and a fast-paced, work-from-home environment
- • Strong interpersonal and communication skills
