- • Serve as the primary point of contact for clients with complex health and social needs, coordinating care across doctors, specialists, pharmacists, social services, and other providers to ensure alignment on care goals.
- • Assess client needs across physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social supports, and housing.
- • Develop, oversee, and update individualized client care plans and SMART goals based on clinical assessments and client input.
- • Facilitate timely and cost-effective access to care through office-based, telehealth, or field-based services, depending on client location and accessibility.
- • Connect clients to community-based services and social supports, including transportation (e.g., ACCESS), housing resources, and behavioral health programs.
- • Advocate on behalf of clients with healthcare professionals, including primary care providers (PCPs), to ensure appropriate services and interventions are delivered.
- • Apply evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles in all client interactions.
- • Conduct outreach and engagement activities to link unconnected individuals to the Enhanced Care Management (ECM) program and document all activities in the Client Relationship Management (CRM) system.
- • Monitor and evaluate client progress toward goals, adjusting care plans as needed and documenting outcomes in compliance with program timelines.
- • Provide mental health promotion and education to clients and their caregivers or family support persons to foster wellness, recovery, independence, and empowerment.
- • Arrange and coordinate transportation services to ensure continuity of care and appointment attendance.
- • Maintain accurate, up-to-date client records in the Electronic Medical Record (EMR) system and other business systems, ensuring compliance with documentation standards.
- • Complete all required monthly reporting to ensure program compliance and regulatory adherence.
- • Attend all mandatory training sessions and professional development programs as assigned by management.
- • Work within designated hours (Monday–Friday, 8:30 a.m.–5:00 p.m. Pacific Time) with flexibility for occasional evenings or weekends as needed.

🎯 Requirements
- • 2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- • Willing and able to work Monday–Friday, 8:30 a.m.–5:00 p.m. Pacific Time with flexibility for evenings and weekends
- • Clean driving record, valid driver's license, and reliable transportation
- • Working knowledge of government and community resources related to social determinants of health
- • Excellent oral and written communication skills and positive interpersonal skills
- • General computer skills and working knowledge of Google Workspace, MS Office, and the internet
