These visits are crucial for understanding their current status and identifying a wide spectrum of needs, including physical health, behavioral health, functional capabilities, psychosocial well-being, financial circumstances, employment prospects, and independent living expectations.
• • Leveraging established criteria, you will be responsible for authorizing appropriate home and community-based services, ensuring these services are effectively delivered and that members' needs are consistently met.

• • Various immunizations and/or associated medical tests may be required to ensure the health and safety of both the coordinator and the members served.
• • The role operates on a standard 8:00 am – 5:00 pm EST schedule, with no on-call responsibilities, offering a predictable work-life balance.
• • After one year of successful performance and with management approval, there is an option to explore a compressed work week schedule, providing further flexibility.
