- • You will collaborate closely with a multidisciplinary team of clinical and social support professionals to comprehensively address the physical, behavioral health, and long-term service needs of each member.
- • A core responsibility involves conducting thorough, objective, and in-person assessments of members in their environment. These visits are crucial for understanding their current status and identifying a wide spectrum of needs, including physical health, behavioral health, functional capabilities, psychosocial well-being, financial circumstances, employment prospects, and independent living expectations.
- • Leveraging established criteria, you will be responsible for authorizing appropriate home and community-based services, ensuring these services are effectively delivered and that members' needs are consistently met.
- • This role demands exceptional problem-solving skills, underpinned by critical thinking, to navigate complex member situations and advocate for necessary resources.
- • Superior time management and prioritization abilities are essential for effectively managing a diverse caseload and responding to various demands from internal and external stakeholders.

- • You will be expected to maintain a valid Driver's License to facilitate home visits and travel within the designated service area.

- • Please note that this position has a 24-month tenure requirement before eligibility to apply for other internal positions, ensuring a foundational understanding and commitment to the role.
- • Various immunizations and/or associated medical tests may be required to ensure the health and safety of both the coordinator and the members served.
- • The role operates on a standard 8:00 am – 5:00 pm EST schedule, with no on-call responsibilities, offering a predictable work-life balance.
- • After one year of successful performance and with management approval, there is an option to explore a compressed work week schedule, providing further flexibility.
