📋 Description

• • The Clinical Associate is an integral part of Devoted Medical, Devoted Health’s interdisciplinary virtual and home-based medical practice, serving as a liaison between healthcare services and members to ensure high-touch, compassionate care coordination.
• • Day-to-day responsibilities include preparing members for telehealth visits, coordinating lab tests, imaging, durable medical equipment, and home health services, managing care gaps, communicating with PCPs and pharmacies, providing patient education, and advocating for members to improve adherence to care plans.
• • Devoted Health is a mission-driven organization founded in 2017 dedicated to improving the health and well-being of older Americans by delivering integrated, family-centered care through a combination of clinical services, health insurance, and technology.
• • In this role, you will develop expertise in care coordination, patient advocacy, and healthcare navigation while gaining hands-on experience with telehealth platforms, electronic health records, and interdisciplinary clinical teams in a fast-growing, purpose-led environment.
