- • Vynca is seeking a dedicated and compassionate Clinical Lead Care Manager (CLCM) to join our mission of providing comprehensive care for more quality days at home. This remote role is pivotal in transforming care for individuals with complex needs, acting as the primary point of contact for clients and ensuring seamless coordination across their entire care ecosystem.
- • As a CLCM, you will be instrumental in managing client cases, meticulously coordinating health care benefits, delivering essential education, and facilitating timely and cost-effective access to care. Your role extends to fostering a collaborative environment, communicating effectively with clients' caregivers, family support persons, and a diverse range of healthcare providers, including doctors, specialists, pharmacists, and social services professionals.
- • Your overarching goal will be to promote wellness, recovery, independence, and resilience among members, empowering them to navigate their healthcare journey. This involves ensuring they have access to appropriate services and maximizing their benefits, all while upholding Vynca's core values of Excellence, Compassion, Curiosity, and Integrity.
- • A significant aspect of your responsibilities will involve conducting thorough assessments of member needs across a broad spectrum of health and social determinants. This includes evaluating physical health, mental health, substance use disorders (SUD), oral health, palliative care, memory care, and trauma-informed care needs. You will also identify and address social support and housing requirements, making crucial referrals and linkages to community-based services and supports.
- • You will take ownership of the development of client care plans and the establishment of clear, achievable goals. This process will be client-centered, ensuring that the care provided is tailored to individual circumstances and aspirations.
- • The CLCM is expected to offer services in locations most accessible and comfortable for the member, whether that be through office-based appointments, telehealth consultations, or field-based visits. This flexibility ensures that barriers to care are minimized.
- • A key function of the role is to act as a strong advocate for the client, liaising with healthcare professionals such as Primary Care Physicians (PCPs) and specialists to ensure their needs are met and understood.
- • You will be expected to utilize evidence-based practices, integrating methodologies like Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles into your daily interactions and care planning.
- • Proactive outreach and engagement activities are essential to facilitate linkage to the Enhanced Care Management (ECM) program. All interactions and progress will be meticulously logged in the Client Relationship Management (CRM) system.
- • Continuous evaluation of client progress is vital. You will regularly update and refine SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals in collaboration with the client, ensuring a dynamic and responsive approach to care.
- • Providing mental health promotion and support is a core component of the role, contributing to the holistic well-being of the client.
- • You will also be responsible for arranging necessary transportation services, such as ACCESS, to ensure clients can attend appointments and access resources.
- • Meticulous and timely documentation is paramount. This includes completing all required documentation, such as outcome measures, within the established timeframes outlined in individual care plans. Maintaining up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems is critical for continuity of care and program compliance.
- • You will be responsible for completing monthly reporting to ensure program compliance and contribute to the overall success of the ECM program.
- • Continuous professional development is encouraged, and you will be expected to attend training sessions as assigned to stay abreast of best practices and program updates.
- • This role requires a commitment to working Monday-Friday, 8:30 am - 5:00 pm Pacific Time, with the understanding that occasional evening and weekend availability may be necessary to meet client needs.
