  • • This position is central to the PACE (Program of All-Inclusive Care for the Elderly) model, requiring a deep understanding of geriatric care and the unique needs of a frail or elderly population.
  • • Your primary responsibility will be the support and seamless implementation of home care services, acting as a critical link between participants, their families, healthcare providers, and external caregiving resources.
  • • You will conduct thorough home visit assessments to evaluate the safety and suitability of the participant's home environment, identifying potential hazards and recommending necessary modifications to ensure a secure living space.
  • • This involves close collaboration with a multidisciplinary Interdisciplinary Team (IDT) and attending scheduled IDT and coordination meetings to contribute insights and ensure holistic care planning.
  • • A key aspect of your role will be coordinating with a network of home caregiver vendors, ensuring they meet AltaMed's high standards and are equipped to provide the necessary support.
  • • You will also work closely with family caregivers, offering guidance, support, and resources to empower them in their caregiving roles.
  • • The coordination of durable medical equipment (DME) is another vital function, ensuring participants have access to the necessary tools and devices to maintain their health and mobility at home.
  • • This includes assessing needs, facilitating procurement, and ensuring proper usage and maintenance of DME.
  • • You will also be responsible for coordinating nursing home care when required, liaising with facilities and ensuring a smooth transition and continuity of care.
  • • Acting as the primary liaison between the IDT and home care providers, you will facilitate clear and consistent communication, ensuring that care plans are understood and effectively executed.
  • • Strong leadership and exceptional communication skills are essential to effectively bridge these different parties and advocate for participant needs.
  • • You must possess the ability to identify and understand participants' complex medical and social needs, translating these into actionable home care requirements.
