  • • As a Clinical Documentation Integrity (CDI) Specialist at Guidehouse, you will play a pivotal role in ensuring the accuracy, completeness, and compliance of clinical documentation within healthcare systems. This remote position offers the opportunity to contribute significantly to patient care quality and financial integrity by meticulously reviewing patient charts and collaborating with healthcare professionals.
  • • Your primary responsibility will involve conducting comprehensive secondary clinical chart reviews. This means you will delve into patient records to identify any potential gaps or ambiguities in the documentation that might lead to missed opportunities for clarification. This proactive approach is crucial for accurately reflecting the patient's condition and the care provided.
  • • You will be a key collaborator, working closely with a multidisciplinary team. This includes Coders, Coding Educators, Coding Quality Auditors, Case Managers, the Quality Department, and most importantly, Providers (physicians and clinicians). Your insights will help ensure that documentation is not only clinically appropriate but also accurately reflects the severity of illness (SOI) and risk of mortality (ROM) for each patient.
  • • A core function of your role is to ensure that documentation aligns with current CMS (Centers for Medicare & Medicaid Services) regulations and other relevant governmental or industry standards. This adherence is vital for compliant coding and appropriate reimbursement.
  • • On a daily basis, you will perform concurrent reviews of inpatient records for your assigned unit(s). This involves assessing the physician and/or clinician documentation to confirm it is complete and accurate at the time of discharge. This timely review is essential for accurate, prompt, and compliant coding processes.
  • • You will also review daily admissions to your assigned unit. As part of this process, you will perform an initial assessment for a working Diagnosis Related Group (DRG) assignment. You will then enter this information, along with follow-up case review details, into the CDI software system or onto a designated worksheet.
  • • A dynamic aspect of the role is updating the “working DRG” as new documentation emerges or as responses to physician queries lead to a change in the DRG assignment. This requires continuous monitoring and analysis of the patient's record.
  • • Effective communication is key. You will need to inform the CDI Coordinator if your daily review assignments become unmanageably high or too low. This allows for efficient workload distribution and ensures that the team can manage its capacity effectively.
  • • A critical skill you will employ is initiating compliant physician queries. When documentation is unclear, ambiguous, or missing essential details, you will craft and submit queries to the physician to seek immediate clarification. You will follow up diligently to ensure a prompt response, guiding your query process with established best practices from organizations like AHIMA (American Health Information Management Association).
  • • This role demands a strong understanding of coding guidelines and the ability to interpret complex medical information. Your work directly impacts the quality of patient care data, the efficiency of the revenue cycle, and the overall compliance of the healthcare organization.
  • • By bridging the gap between clinical practice and coding, you will contribute to a more transparent and accurate healthcare data landscape. Your efforts will help ensure that healthcare providers are appropriately recognized for the complexity of care they deliver, ultimately supporting better patient outcomes and operational efficiency.
