• • The Clinical Guide: Diabetes Educator (Certified Diabetes Care and Education Specialist) role at Devoted Health, Inc. is a fully remote position focused on improving health outcomes for members with uncontrolled diabetes through education, coaching, and care coordination.
• • Day to day, the Clinical Guide will screen members for eligibility, conduct comprehensive assessments, deliver ongoing diabetes education across key areas (medications, glucose monitoring, lifestyle), schedule follow-ups, connect members with internal resources, set SMART goals, use motivational interviewing to drive behavior change, identify care gaps, collaborate with a multidisciplinary team, and maintain accurate documentation.
