  • • The Clinical Intake Coordinator is a pivotal role responsible for ensuring a smooth and successful onboarding experience for members into our care programs. This position is critical in collecting comprehensive and accurate intake information, meticulously preparing each case for a seamless transition to the dedicated care team. You will be the primary point of contact for members, engaging them directly to gather and validate essential intake documentation. Your efforts will directly contribute to delivering high-quality handoffs that empower Care Guides to commence ongoing patient care without the need for subsequent follow-up, thereby optimizing efficiency and patient satisfaction.
  • • This role operates in close collaboration with our Care Guides, Member Growth, and Care Operations teams, working in concert to expedite the movement of members through the intake pipeline. The Clinical Intake Coordinator reports directly to the Care Guide Clinical Manager, ensuring alignment with clinical oversight and operational goals.
  • • **Member Engagement & Intake:**
  • • Proactively initiate contact with members through various channels, including phone calls, SMS messages, and email, to systematically collect all required clinical intake information.
  • • Act as the main liaison for all patient inquiries, providing clear, empathetic, and informative communication throughout the entire intake journey.
  • • Diligently gather and meticulously validate all necessary patient demographic, clinical, and treatment-related information essential for initiating care.
  • • **Care Coordination & Scheduling:**
  • • Expertly schedule and confirm patient treatment appointments, ensuring seamless coordination with external nursing agencies and designated infusion providers.
  • • Confirm the successful receipt of prescribed medications and ensure patients are thoroughly informed about all treatment preparation requirements well in advance of their scheduled therapy.
  • • Prepare and deliver comprehensive handoff packages to Care Coordinators, which include detailed intake summaries, all supporting documentation, and clearly defined next steps for ongoing care.
  • • **Insurance & Authorization Support:**
  • • Conduct thorough verification of member eligibility and benefits, confirming insurance coverage details, specific payer information, and any applicable cost-sharing requirements necessary for initiating care.
  • • Actively support the preparation of prior authorization documentation and diligently track the status of authorizations, including renewals and extensions, to prevent any delays in care.
  • • **Operational Excellence & Compliance:**
  • • Promptly identify and escalate any urgent clinical or patient safety concerns to licensed clinical staff in strict accordance with established protocols.
  • • Maintain the highest level of accuracy and completeness in all documentation across our CRM and EHR systems, ensuring seamless continuity of care and reliable data.
  • • Adhere rigorously to all HIPAA compliance regulations and member privacy standards in every aspect of communication and record-keeping.
