- • As a Clinical Intake Specialist within our Enhanced Care Management (ECM) program, you will play a pivotal role in bridging the gap between newly referred clients and the comprehensive support services offered by our primary care management team. This position is central to our mission of enabling individuals to live more quality days in the comfort of their homes. Your primary responsibility will be to engage directly with clients, meticulously understand their unique circumstances and needs, and expertly connect them with the most appropriate resources and care pathways.
- • The ideal candidate for this role is someone who possesses exceptional interpersonal skills, projecting a personable and professional demeanor over the phone. You will be adept at swiftly reviewing medical charts, demonstrating a keen ability to comprehend diverse medical conditions and their implications for patient care. A person-centered approach is paramount, ensuring that every interaction is tailored to the individual's specific needs and preferences. You must be adept at prioritizing tasks efficiently to maximize client engagement and ensure timely access to care.
- • Your daily responsibilities will include conducting proactive outreach to newly referred clients, meticulously logging all activities within our Salesforce CRM system to maintain accurate records and track progress. You will be expected to conduct a significant volume of intake visits daily, typically between 8 to 12, ensuring that each assessment is both efficient and thorough. This involves guiding clients through the completion of initial intake forms, including crucial psychosocial assessments and the collaborative development of care plans with clearly defined goals of care.
- • A critical aspect of your role will involve the careful review of client medical charts. Through this review, you will identify and document key medical needs, potential risks, and suggest appropriate interventions to the care team. This information is vital for developing personalized and effective care strategies. You will be expected to communicate critical information and client preferences clearly and concisely to the broader care team, ensuring a holistic and coordinated approach to care planning.
- • In alignment with Vynca's commitment to evidence-based practices, you will be encouraged to utilize methodologies such as Motivational Interviewing to foster client engagement, Harm Reduction strategies to mitigate risks, and Trauma-Informed Care principles to ensure sensitive and respectful interactions. Your dedication to these principles will significantly contribute to building trust and rapport with our diverse client population.
- • To ensure seamless team operations and continuous improvement, you will participate in weekly Team Operations Meetings, contributing insights and collaborating on strategies to enhance service delivery. Maintaining a highly structured and organized schedule is essential for optimizing client intake processes and maximizing engagement opportunities. This includes proactively reaching out to clients who may have missed appointments or have not yet been contacted, ensuring no one falls through the cracks.
- • You will also be on standby to receive and manage warm transfers from field teams, guaranteeing immediate, same-day responses to urgent client needs. This role demands the ability to thrive in a fast-paced, dynamic environment where meeting key performance metrics is crucial for the success of our program and the well-being of our clients.
