This role is integral to our Enrollments team, serving as the primary point of contact for prospective clients seeking our Enhanced Care Management (ECM) services.
- • Your core responsibility will be to proactively engage with potential clients, providing them with comprehensive information about Vynca's innovative services. This involves making a significant volume of outbound calls daily, typically between 60 to 100, to introduce our programs and assess their suitability. You will be instrumental in guiding individuals through the initial stages of enrollment, ensuring they understand how our services can positively impact their lives and the lives of their loved ones.
- • A key aspect of this role is conducting thorough needs assessments. You will meticulously review medical charts to gain a deep understanding of each client's unique health status, documenting their specific needs and identifying potential areas where our care management team can provide the most effective interventions. This analytical approach ensures that we are offering tailored solutions that truly address the complexities of their care requirements.
- • Beyond initial contact, you will play a vital role in supporting the development of personalized care plans. This involves effectively communicating essential information and client preferences gathered during outreach and assessment to the dedicated care management team. Your insights will directly contribute to creating care plans that are not only clinically sound but also deeply aligned with the client's personal goals and values.
- • You will also be responsible for assisting clients in completing psychosocial assessments. This requires a sensitive and empathetic approach, helping individuals articulate their needs and preferences. Furthermore, you will collaborate with clients to develop care plans that outline clear, actionable goals, fostering a sense of partnership and empowerment in their healthcare journey.
- • In all client interactions, you are expected to apply evidence-based practices that promote positive outcomes and respect client autonomy. This includes leveraging techniques such as Motivational Interviewing to encourage engagement and behavior change, Harm Reduction strategies to minimize risks, and Trauma-Informed Care principles to ensure a safe and supportive environment for all individuals, particularly those with complex needs.
- • Maintaining accurate and detailed records is paramount. You will diligently record all client interactions, assessments, and progress notes within our Client Relationship Management (CRM) system. This ensures continuity of care, facilitates effective team communication, and provides valuable data for service improvement.
- • Collaboration is central to Vynca's success. You will actively participate in weekly Team Operations Meetings, working alongside colleagues to coordinate care delivery, share insights, and collectively optimize our service offerings.
