• • As Director of Operational Performance & Population Analytics at Chamber Cardio, Inc., you will own the critical link between data and action by leading performance tracking and population analytics to ensure care programs deliver measurable clinical and operational outcomes. This role is not about reporting or retrospective claims analysis—it’s about turning insights into accountability, driving improvement, and partnering with clinical and operations leadership to shape what gets built and how it performs.
• • Day to day, you will own leading indicators, SLA tracking, quality measures, and program ROI; build and maintain real-time dashboards for Care Ops and Clinical leadership; analyze attributed populations to surface risk concentrations and care gaps; design and run QA audit programs for human and AI-enabled workflows; conduct root cause analysis when targets are missed; and partner cross-functionally to translate data into actionable improvement plans with closed-loop accountability.
