- • The Document Creation and Management Chart Integrity Analyst ensures the accuracy, completeness, and uniqueness of patient records across Trinity Health’s electronic health record (EHR) systems by identifying, investigating, and resolving wrong encounters, overlays, and documentation errors in both acute and ambulatory settings.
- • Day-to-day responsibilities include managing Epic Chart Correction InBasket submissions, reviewing and resolving chart correction cases, collaborating with clinical and administrative teams to correct patient identification errors, updating records with alerts and advisories, validating corrections, and supporting 24/7 operations through rotating weekend and holiday shifts.
- • The role is part of the TogetherCare HIM Operations Team, working closely with Data Integrity Liaisons, Health Informatics, clinicians, Registration, Billing, and ancillary departments to maintain data integrity and support patient safety, compliance, and accurate reimbursement across Trinity Health’s nationwide network.
- • The analyst will develop deep expertise in EHR data integrity, healthcare workflows, Epic system navigation, HIPAA and Joint Commission compliance, and cross-functional problem-solving—gaining advanced analytical and operational skills critical to healthcare data quality and patient safety initiatives.
🎯 Requirements
- • High school diploma or GED required; Associate’s degree in Health Information Management, Computer Science Informatics, Information Technology, or related field preferred.
- • Minimum five (5) years of extensive healthcare documentation or HIM experience in acute, ambulatory, or physician office settings with demonstrated analytical and project planning skills.
- • Advanced proficiency in Microsoft Windows, web applications, EHR systems (specifically Epic), and electronic resources; knowledge of clinical workflows, medical record content, registration, and billing processes.
- • Ability to work independently in a remote setting with minimal supervision, manage multiple priorities, and adapt to changing demands in a 24/7 operational environment.
- • Excellent interpersonal and communication skills to effectively engage with clinicians, leadership, and administrative staff across all levels of the organization.
- • Demonstrated ability to interpret and apply HIPAA and Joint Commission standards to document creation and management operations.
