- • Serve as a Family Mentor on a multidisciplinary care team to support caregivers of patients receiving virtual eating disorder treatment through Equip Health Inc.
- • Conduct individual and group skills-based sessions with caregivers to help them integrate into and adhere to evidence-based treatment plans.
- • Implement motivational strategies to increase caregiver engagement with care plans, including monitoring completion of program elements and addressing barriers to adherence.
- • Proactively reach out to caregivers via phone and messaging to recover missed appointments, provide reminders, and ensure consistent participation in treatment.
- • Maintain an availability calendar to allow caregivers to schedule appointments and coordinate care team interactions.
- • Identify and address day-to-day challenges caregivers face by providing tangible solutions such as customized grocery lists, resource referrals, and coordination of additional sessions.
- • Guide caregivers in developing advocacy-related documentation, including 504 Plan accommodation request letters.
- • Participate in multidisciplinary treatment team huddles to align on patient progress and coordinate care strategies.
- • Build trusting, respectful relationships with caregivers through active listening and guidance in coping-ahead strategies.
- • Complete timely, compliant documentation of all caregiver interactions in accordance with Equip’s policies and procedures.
- • Attend mandatory internal meetings including supervision, treatment huddles, labs, and all-hands sessions.
- • Engage in ongoing training on evidence-based treatments including Family-Based Treatment, Enhanced Cognitive Behavioral Therapy, and Cognitive Behavioral Therapy for ARFID.
- • Support patient retention and satisfaction by personalizing care plans to meet unique family needs and ensuring access to appropriate services.
- • Work remotely with consistent availability for 33 hours per week of direct patient care, including evenings until 8pm two nights per week.
- • Maintain a minimum of 20 hours per week of direct patient care after 2pm in the patient’s respective time zone.
- • Operate standard office equipment and video conferencing tools while working from home with camera enabled.
- • Navigate ambiguity and adapt quickly in a fast-paced, fluid virtual care environment.
- • Collaborate with internal teams to ensure seamless delivery of care and continuous improvement of caregiver support systems.
🎯 Requirements
- • 2+ years of experience working with families or supports in a healthcare setting (e.g., coaching, case management, mentorship, social work, or milieu therapy)
- • Lived experience as an eating disorder support for a loved one in firm/stable recovery
- • Ability to connect with diverse individuals across varying lived experiences
- • Strong oral and written communication skills with ability to build professional, collaborative relationships
- • Comfortable working remotely and using digital platforms, including being on camera during sessions
- • Strong organizational and time management skills with a proven ability to meet goals and maintain attention to detail
