📋 Description

• The Family Mentor is an integral member of a patient’s multidisciplinary care team, ensuring caregivers successfully integrate into Equip treatment by adhering to care plans and fully engaging in treatment.
• Day-to-day responsibilities include providing skills-based individual and group sessions to caregivers, implementing motivational strategies, conducting proactive outreach, guiding caregivers in developing advocacy documentation, maintaining availability calendars, and completing timely documentation of interactions.
• Equip Health Inc. is a fully virtual, evidence-based eating disorder treatment program operating in all 50 states, recognized as one of Time’s most influential companies of 2023, dedicated to empowering individuals and families to reach lasting recovery through personalized, compassionate care.
• In this role, you will build expertise in evidence-based treatments like Family-Based Treatment and Enhanced CBT, develop advanced coaching and advocacy skills, and contribute to a mission-driven culture focused on diversity, equity, inclusion, and belonging while supporting thousands of patients and families nationwide.

🎯 Requirements

• 2+ years of experience working with families/supports in a health care setting including coaching, case management, mentorship, social work, or milieu therapy
• Lived experience as an eating disorder support for a loved one in firm/stable recovery
• Ability to connect with a range of diverse people with varying lived experiences
• Strong oral and written communication skills with the ability to build effective, professional, and collaborative relationships
• Strong organizational and time management skills with a proven track record of meeting goals
• Comfortable working remotely, including being on camera, and learning new technologies

• Earning Potential Range: $54,000 to $65,000 Base Pay + 15% Annual Performance-Based Bonus Potential
