• • The Home Health and Personal Care Aid role is essential to delivering compassionate, in-home support to elderly adults, individuals with disabilities, and those recovering from illness or injury across New York City. As a direct caregiver, you will make a meaningful difference in clients’ daily lives by providing hands-on assistance that promotes safety, dignity, and well-being.
• • Day to day, you will assist clients with personal care activities such as bathing, dressing, grooming, and toileting; support mobility and transfers; prepare meals and assist with feeding; perform light housekeeping and laundry; accompany clients to appointments or help with errands; monitor and report changes in condition to care coordinators; provide companionship and emotional support; and maintain accurate care logs and documentation.
