- • Administer IV infusion medications safely and effectively in patients’ homes as part of specialty pharmacy care.
- • Collaborate with pharmacists and interdisciplinary care teams to ensure comprehensive, holistic patient well-being.
- • Conduct thorough patient assessments, document treatments, and record clinical progress to maintain accurate and compliant patient records.
- • Serve as the primary point of contact for patients, providing updates, answering questions, and coordinating care across the care continuum.
- • Exercise clinical autonomy in decision-making to optimize patient outcomes and ensure adherence to treatment protocols.
- • Perform multiple patient visits per week, including scheduled evening and weekend visits as needed to meet patient care demands.
- • Maintain proficiency in IV insertion, infusion techniques, and sterile procedure standards in home-based settings.
- • Travel within a large geographic region to reach patient residences, requiring reliable transportation and consistent scheduling.
- • Ensure compliance with all regulatory, safety, and documentation standards for home infusion nursing practices.
- • Utilize secure, high-speed internet (minimum 10Mbps download / 5Mbps upload) when performing remote documentation or telehealth coordination from home.
- • Adapt to variable patient schedules and complex medical needs while maintaining compassionate, patient-centered care.
- • Uphold confidentiality and ethical standards in all patient interactions and record-keeping practices.
🎯 Requirements
- • Active RN license in the state of practice
- • Minimum 2 years of RN experience
- • At least 1 year of experience in critical care, acute care, or home healthcare
- • Proficiency in IV insertion and infusion techniques
- • Valid driver’s license and ability to travel within a large geographic region
- • Ability to perform multiple patient visits per week, including evenings and weekends
