- • Coordinate and deliver medical services to home-bound patients through the Home Visit Program, with a primary focus on Medicare Annual Wellness Visits.
- • Provide comprehensive care coordination for complex, high-risk client cases requiring home-based medical attention.
- • Ensure all home visits adhere to professional medical standards and comply with regulatory requirements for patient care.
- • Collaborate with clinical teams to develop and implement individualized care plans for patients with chronic or complex health conditions.
- • Conduct patient assessments during home visits to monitor health status, identify emerging medical needs, and adjust care strategies accordingly.
- • Document all patient interactions, clinical findings, and care interventions accurately in electronic health records in compliance with organizational and regulatory protocols.
- • Communicate effectively with patients, families, and healthcare providers to ensure continuity of care and patient understanding of treatment plans.
- • Facilitate referrals to specialists, social services, or community resources as needed to support holistic patient care.
- • Maintain strict adherence to infection control, safety, and privacy protocols during all home visits.
- • Participate in team meetings, case reviews, and quality improvement initiatives to enhance program effectiveness and patient outcomes.
- • Manage scheduling and logistics for home visits to ensure timely and efficient service delivery across assigned geographic areas.
- • Serve as a point of contact for patients and caregivers regarding medical concerns, appointment changes, or care instructions.
- • Support program expansion and operational efficiency by providing feedback on workflow, patient needs, and resource allocation.
- • Maintain current knowledge of Medicare guidelines, home health regulations, and best practices in home-based care.

🎯 Requirements
- • High School graduate or equivalent with a Medical Assistant certification or equivalent required
- • Minimum of 2 years of back-office experience preferred
- • Minimum of 2 years of experience in a managed health care environment (IPA, HMO, or health plan) preferred
- • Experience in hospice, palliative care, or high-risk care settings preferred
- • Bilingual in Spanish and English preferred
- • Valid BLS certification or higher from American Heart Association (AHA) or American Red Cross required
