📋 Description
- • The Integrated Duals Program Coordinator II supports care coordination and engagement activities while serving as an advanced resource across multiple teams, ensuring timely, effective, and member-centered services.
- • Day-to-day responsibilities include acting as the first point of contact for complex member and provider escalations, mentoring Program Coordinator I staff, maintaining accurate documentation in electronic systems, coordinating scheduling and outreach efforts, and identifying social support needs to initiate nonclinical referrals.

- • In this role, the individual can develop leadership and mentoring skills, deepen expertise in healthcare coordination and compliance, influence process improvements, and serve as a trusted resource for complex care coordination across teams.
🎯 Requirements
- • High School diploma or GED required
- • Three (3) years of experience in healthcare required
- • Intermediate proficiency with Microsoft Office (Outlook, Word, Excel)
- • Ability to communicate effectively with diverse individuals and strong organizational and time management skills
- • Knowledge of local, state, and federal healthcare laws and regulations and company policies regarding case management practices
