- • Vynca is seeking a dedicated and compassionate Clinical Lead Care Manager (CLCM) to join our mission of providing comprehensive care for more quality days at home. This pivotal role involves serving as the primary point of contact for clients with complex needs, ensuring seamless coordination across their entire care ecosystem.
- • As a CLCM, you will be instrumental in bridging the gap between clients, their families, and a diverse network of healthcare providers, including physicians, specialists, pharmacists, and social services professionals. Your expertise will ensure that all parties are aligned on the client's needs and care plan, fostering a collaborative environment focused on the client's well-being.
- • You will manage client cases with a high degree of autonomy and responsibility, meticulously coordinating health care benefits to maximize client access to services in a timely and cost-effective manner. This includes educating clients and their support systems about available benefits and navigating the complexities of the healthcare system.
- • A core aspect of this role is promoting wellness, recovery, independence, and resilience among members. You will empower clients by facilitating their access to appropriate services and resources, acting as a strong advocate on their behalf with healthcare professionals and other providers.
- • The CLCM will conduct thorough assessments of member needs, encompassing a wide spectrum of care areas. This includes evaluating physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, and trauma-informed care. Furthermore, you will identify and connect clients to essential social supports and community-based services, recognizing the critical impact of social determinants of health on overall well-being.
- • You will be responsible for overseeing the development of individualized client care plans and setting achievable SMART goals, utilizing evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles to guide your approach.
- • This role requires proactive outreach and engagement activities to facilitate linkage to the Enhanced Care Management (ECM) program. You will meticulously log all activities in the Client Relationship Management (CRM) system, ensuring accurate and up-to-date records.
- • A key responsibility is to evaluate client progress regularly, making necessary adjustments to care plans and goals to ensure continued progress towards recovery and independence.
- • You will provide crucial mental health promotion support and assist with practical needs, such as arranging transportation (e.g., ACCESS), to remove barriers to care.
- • Meticulous documentation is paramount. You will complete all required documentation, including outcome measures, within established timeframes. Maintaining up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems is essential for continuity of care and program compliance.
- • Monthly reporting will be a key component of this role, ensuring adherence to program guidelines and demonstrating the impact of your work. You will also be expected to attend all assigned training sessions to stay abreast of best practices and program updates.
- • This position offers the flexibility of remote work within the United States, with specific operational hours of Monday-Friday, 8:30 am - 5:00 pm Pacific Time, with the potential for occasional evening and weekend work to meet client needs.
