- • Join Vynca as an LVN Case Manager (Clinical Lead Care Manager) to serve as the primary point of contact for clients with complex needs, coordinating care across providers and ensuring access to timely, cost-effective services.
- • Assess member needs across physical, mental, oral health, palliative care, trauma-informed care, social supports, housing, and community linkages; develop and update care plans with SMART goals.
- • Work within a mission-driven team guided by core values of Excellence, Compassion, Curiosity, and Integrity, focused on transforming care for individuals to enable more quality days at home.
- • Gain experience in care coordination, advocacy, evidence-based practices (Motivational Interviewing, Harm Reduction), and healthcare systems while supporting vulnerable populations in a remote, multidisciplinary environment.
