📋 Description
- • Serve as the primary care provider for entire families, delivering comprehensive primary, acute, chronic, and pediatric care in home and virtual settings (telehealth via video, phone, SMS) to adult and pediatric patients.
- • Manage a longitudinal panel of families, focusing on preventive care, care coordination, and whole-family health outcomes through consistent, culturally congruent, and non-judgmental clinical engagement.
- • Collaborate closely with an interdisciplinary clinical team including physicians, behavioral health counselors, registered dietitians, psychiatrists, RNs, and upskilled medical assistants to deliver integrated physical, behavioral, and social care.
- • Utilize Nest Health’s electronic health record and care management platforms to document patient interactions, track tasks, organize clinical data, and communicate with team members, patients, and community resources.
- • Attend daily team huddles and weekly case conferences to advise on patient health needs, co-develop Nest Action Plans (NAP), and align care goals across disciplines.
- • Diagnose and treat chronic and infectious diseases, prescribe and reconcile medications, order and interpret diagnostic tests, and administer necessary clinical procedures including venipuncture and phlebotomy.
- • Educate patients and families on condition management, medication adherence, prevention strategies, and eligibility for health and social services, acting as an advocate and care navigator.
- • Participate in outreach initiatives to explain Nest Health’s services to potential families and support program enrollment.
- • Maintain compliance with all clinical protocols, documentation standards, and safety policies while delivering care in diverse home environments.
- • Operate as a fully autonomous clinical provider with high accountability, requiring self-directed learning, proactive identification of system gaps, and clear communication of challenges or opportunities to the team.
- • Travel extensively to patient homes across the New Orleans metro area, carrying portable medical supplies and adhering to Nest’s Motor Vehicle Safety Policy.
- • Maintain current BLS/CPR certification and adhere to all state and federal licensing, documentation, and clinical practice standards.
- • Participate in a rotating call schedule that includes occasional evening and weekend hours each month to ensure continuous patient access.
- • Contribute to iterative improvements in care delivery by providing feedback on workflows, tools, and team processes within a fast-paced startup environment.
- • Demonstrate flexibility and adaptability in a dynamic startup setting where roles may evolve rapidly and require engagement in multiple functions beyond traditional NP responsibilities.
