- • As a Nurse Practitioner (NP) at Nest Health, you will be at the forefront of a revolutionary value-based care model, serving as the primary care provider for families in the Kingman, Arizona area. This is a unique opportunity to deliver comprehensive, in-home primary, acute, and chronic care to both adult and pediatric patients, including newborns, directly within their homes. You will be instrumental in improving whole-family health through longitudinal care, preventive services, and proactive care management, leveraging telehealth and virtual modalities like video, phone, and SMS to extend your reach and impact.
- • You will operate as a crucial health practitioner and educator, delivering care with cultural sensitivity and a non-judgmental approach. Your role involves close collaboration with a robust interdisciplinary team, including physicians, licensed clinical social workers (LCSWs), registered nurses (RNs), psychiatrists, registered dietitians, and medical assistants who are upskilled to assist with social services navigation. This team-based approach ensures holistic care for every family.
- • A significant aspect of your role will be managing a dedicated panel of families, acting as their consistent point of contact for all their primary care needs. This longitudinal relationship allows for a deeper understanding of family dynamics and health trajectories, enabling more personalized and effective care plans.
- • You will utilize Nest Health’s advanced electronic health record (EHR) and care management tools to meticulously collect data, document all member interactions in the field, organize critical information, track essential tasks, and facilitate seamless communication with your team, patients, and relevant community resources. This digital infrastructure is key to our efficient and coordinated care delivery.
- • Daily engagement in team huddles and weekly case conferences is expected. In these forums, you will provide essential input on physical health needs, contribute to the establishment of health goals, and actively participate in developing the NAP (Nest Action Plan) for each member, ensuring a unified and strategic approach to care.
- • Your clinical responsibilities will include proposing evidence-based treatments for both chronic and infectious diseases, prescribing and reconciling medications accurately, and providing clear, concise education to patients and their families about their conditions, treatment plans, and prescribed medications. Empowering patients with knowledge is a cornerstone of our philosophy.
- • You will be responsible for ordering, administering, and recording diagnostic tests, ensuring that all necessary assessments are conducted to inform diagnosis and treatment. This includes proficiency in clinical nursing skills such as venipuncture and phlebotomy, as well as conducting thorough patient assessments.
- • A vital part of your role involves acting as an advocate and navigator for Nest families. You will provide crucial information and assistance, guiding them through the complexities of the healthcare and social services landscape, and educating them on the availability and eligibility criteria for various health and social care programs. This advocacy ensures families can access the support they need beyond direct medical care.
- • You will also participate in proactive outreach efforts to engage potential Nest families, clearly explaining the program's benefits and services to encourage enrollment and access to our innovative care model.

- • This is a full-time, five-day work week position that includes participation in a rotating call schedule, which may involve occasional evenings and weekend days each month, ensuring continuous support for our families.
