- • As a Nurse Practitioner or Physician Assistant in Alignment Healthcare’s Care Anywhere program, you will deliver in-home assessments and clinical support to seniors and chronically ill patients, directly contributing to reduced hospitalizations and improved health outcomes in the Stockton, CA area.
- • Your day-to-day responsibilities include conducting comprehensive in-home patient assessments, performing medication reviews and health screenings, providing personalized patient education, identifying diagnoses for care management, collaborating with Regional Medical Officers and care teams to develop individualized care plans, coordinating with PCPs, specialists, and ancillary providers, and leading a broader clinical team that may include nurses, health coaches, social workers, and care coordinators.

🎯 Requirements
- • Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
- • Active Nurse Practitioner or Physician Assistant license
- • Valid Driver License and current automobile insurance
- • NPI Number, DEA registration, and valid BLS certification
- • Ability to communicate positively, professionally, and effectively with patients and team members
- • Strong problem-solving, organizational, and time management skills for fast-paced field work
