• • As a Registered Dietitian with Coram CVS Specialty Infusion Services, you will be at the forefront of shaping a more connected, convenient, and compassionate health experience for individuals.
• • You will play an instrumental role in making a real difference by improving the quality of life for patients in need, working with a passionate team that prioritizes safety and quality in everything they do.
• • This remote role offers the opportunity to work full-time hours from your home, Monday through Friday, typically between 8 AM and 5 PM CST, with a coverage area across the central states.
• • Your primary responsibility will be to coordinate all aspects of patient home nutrition therapy, interacting with a diverse range of internal and external customers including patients, caregivers, case managers, physicians, pharmacists, outpatient dietitians, and vendor representatives.
• • You will leverage your excellent written and verbal customer service skills, alongside advanced computer proficiency, to effectively engage with key partners and patients.
• • A core function of this role involves evaluating and customizing parenteral and enteral prescriptions. This includes meticulous assessment of formula composition (macro and micronutrients), prescribed volume, administration schedules, and other critical parameters, all tailored to each unique patient's specific needs.
• • You will proactively conduct outbound calls to patients and caregivers for ongoing assessments, monitoring their status, tolerance to prescribed therapy, and providing essential education related to their home nutrition support.
• • A crucial part of the onboarding process for new nutrition therapy patients involves reviewing their medical documentation to ensure a smooth transition and comprehensive understanding of their health history.
• • You will also review clinical documentation to assist with the qualification and documentation of medical necessity for reimbursement purposes, working with both Medicare and commercial payers.
• • Developing a patient-specific plan of care is paramount, requiring close coordination with internal and external medical professionals to ensure optimal patient outcomes and adherence to treatment.
• • You will collaborate with the sales team, serving as a content expert to develop and maintain strong customer relationships within the nutrition sector, supporting sales activities with your clinical knowledge.
• • You will be the first point of contact for incoming calls from patients, caregivers, and other disciplines across the pharmacy, adeptly resolving their questions and issues related to nutrition therapy.
• • Training for this position is conducted entirely remotely, ensuring a seamless integration into the team from the comfort of your home.
