- • This role provides Social Work support to Devoted Health’s virtual Palliative Care Program, serving members with complex needs and life-limiting illness who otherwise would not have access to specialized palliative care.
- • Day to day, the Palliative Care Social Work Case Manager conducts telephonic and video visits with patients and caregivers, establishes trust, conducts psychosocial assessments, creates personalized care plans, provides emotional and caregiver support, assists in navigating complex family/support issues, completes goals of care conversations and advanced care planning, conducts long term care planning and placements, places/follows through on behavioral health referrals, investigates concerns for patient abuse or neglect, balances multiple competing priorities, works with family members/caregivers to help them understand member needs, manages or leads conversations regarding goals of care and social drivers of advanced care planning, facilitates and participates in family meetings and joint visits, participates in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development, builds relationships with key local market and internal stakeholders, and documents all clinical interactions in the electronic health record.
- • Devoted Health’s virtual Palliative Care program serves members with complex needs and life-limiting illness, providing highly compassionate, patient-centered goals of care counseling and symptom management for members projected to be in the final year of life, meeting patients where they are and supporting them in defining quality of life, achieving meaningful goals, and transitioning to hospice care.
- • In this role, the person can learn and achieve expertise in palliative and hospice social work, deepen skills in goals of care conversations and advanced care planning, strengthen interdisciplinary collaboration in a virtual care setting, develop proficiency in addressing socioeconomic and psychosocial barriers, and contribute to a mission-driven organization dedicated to improving the health and well-being of older Americans by treating members like family.
🎯 Requirements
- • Master’s degree in social work and active license
- • Minimum of 2 years expertise in palliative or hospice with proficiency in leading goals of care conversations, assisting with advanced care planning, and supporting people through grief, loss, and bereavement
- • Certified hospice and palliative social work credential, or willingness to obtain within one year of employment
- • Comfortable with technology and a fast-paced environment
- • Self-starter with ability to succeed in a work-from-home environment
- • Strong interpersonal and communication skills
- • Comfortable working with patients over video and phone
